Why Biofeedback Produces Short-Term Improvement — Then the GAD Returns
Generalised Anxiety Disorder is a chronic, persistent, pervasive activation of the fear-response mechanism. The amygdala and HPA axis are calibrated to run at a perpetually elevated set-point — producing constant background anxiety, worry, and physiological tension that is not triggered by specific events but simply present as the baseline state.
HRV biofeedback can shift the autonomic balance temporarily toward parasympathetic activity — reducing the physiological expression of that chronic activation. It feels like improvement. GAD scores improve during the treatment period. Then the practice stops, or the patient's life demands prevent regular sessions, and the set-point reasserts. The mechanism was never touched.
