Abstract

For decades, psychiatric and psychological practice has routinely diagnosed fear disorder sufferers with comorbid clinical depression, subjecting them to polypharmaceutical regimens and prolonged therapeutic intervention on the basis of a diagnostic framework this paper argues is both scientifically unfounded and commercially motivated. Drawing on thirty years of longitudinal clinical outcomes across a cohort exceeding 650,000 individuals, this paper presents the case that what mainstream psychiatry classifies as comorbid clinical depression in primary fear disorder sufferers is, in the overwhelming majority of cases, secondary situational distress: the predictable and non-pathological psychological consequence of sustained entrapment in a hyperactive fear response loop. The systematic misclassification of this secondary distress as independent Major Depressive Disorder has been driven not by objective neurobiology but by the structural incentives of the DSM diagnostic checklist system, pharmaceutical market expansion, and insurance billing requirements. The consequences — including inappropriate polypharmacy, prolonged suffering, iatrogenic harm, and the suppression of effective root-cause intervention — represent one of the most significant and least acknowledged public health failures of the modern era.
